The consultations will be held in the following forms:
 
1) direct – a paper form available at the Town Office of Mikołów – Customer Service Office or at the village commune center (Mikołów Mokre, Miodowa 2 street),
 
2) electronic – by downloading a form from Town Office of Mikołów at http://mikolow.eu and the Public Information Bulletin http://www.bip.mikolow.eu in the “Konsultacje społeczne” tab. Please send a scan of the form filled in to um@mikolow.eu or zagospodarowanie.przestrzenne@mikolow.eu.
 
Authorized citizens can cast filled in and signed paper forms in ballot boxes in the following places:
 
1) Town Office of Mikołów – Customer Service Office (Mikołów, Rynek 16),
2) Mokre Village Commune Center (Mikołów Mokre, Miodowa 2 street).
 
Consultations will be held in the period of Sep. 8, 2017 to Sep. 22, 2017.
