Dear All,

If you?d like to have a Mikołów Citizen Card, and you don?t have enough time during the week to apply for, you can still do it on Saturday, September 2 between 9.00am and 2.00pm at the Main Square in Mikołów.
It?s just enough to fill in an application for the card and present your id card with one of the following documents:

– the first page of your PIT form for the previous year with a stamp of the Revenue Office in Mikołów or a copy of the document. If your address is mentioned on the next page of the form you should present that page too.
– the Official Confirmation of Receipt (OCR) issued by the Electronic Incoming Correspondence Box together with the first page of the PIT form for the previous year submitted online and generated document number matching the OCR document identification,

– a certification from the Revenue Office in Mikołów proving that the PIT form for the previous year was submitted.

– pensioners can present the first page of the PIT-40A/11A form the Social Insurance Institution or the Agricultural Social Insurance Fund.

Who can receive a Mikołów Citizen Card?
The Card can be given to any person over 18 years of age living in Mikołów, settling their income tax in the Revenue Office in Mikołów. It can also be given to a child of the above mentioned person or a child in the legal care of the above mentioned person, however, the period the card is granted for lapses when the child turns 18 or 26 in the case of students.
